Copy of Health Records – you may request a copy of your health record at any time. We charge $15-100 or more for a copy of your health record depending on the volume of information and administrative time required. Please allow 30 days for your request to be processed. However, we may get have it available sooner.  

Consent to Release Records – HIPAA regulations require us to receive written consent from you, prior to sharing your confidential health information with any outside party and can be faxed to 888-503-5806. If you would like to send records from our practice to another provider, please complete and return a Consent to Release Records form.

 

Lab Results – Copies of patient lab results are made available to a patient in their portal, only after the ordering doctor has reviewed them.
